Celina, OH: The employer failed to ensure the Bliss punch press (model 41-A) was adequately guarded. Employees operating and/or employees walking/working near the press were exposed to struck-by hazards during fabrication operations.

Celina, OH: The employer failed to ensure the point of operation on the Accurpress 717512 Break Press was adequately guarded. Employees working on or near this machine were exposed to crushing hazards at the point of operation. (b) H&S Company, Inc. on or about September 24, 2012, at the address of 721 Harris Rd. Celina, OH: The employer failed to ensure the point of operation on the Bliss punch press (model 41-A) was adequately guarded. Employees operating this machine were exposed to crushing hazards at the point of operation where the hydraulic ram contacts the metal stock.
