29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Job site, 8858 University Boulevard Avenue, Coraopolis, PA: On or about September 24, 2012, during metal stud framing activities on a mezzanine, the employer did not ensure that fall protection systems/devices were used where fall hazards of up to approximately 8 3/4 feet existed.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage, sharp corners and projections were not avoided and where flexible cords and cables passed through doorways or other pinch points, protection against damage was not provided: a) Job site, 8858 University Avenue, Coraopolis, PA: On or about September 24, 2012, during metal stud installation activities, the employer did not ensure that protection against damaging an extension cord was provided. The cord, used for the saw, ran through a doorway opening, but was not protected against having the cord pinched between the door and the door frame.
