29 CFR 1926.501(b)(4)(iii): Each employee on a walking/working surface was not protected from objects falling through holes (including skylights) by covers. (a) Moravian House Antiques, Roof - On or about September 24, 2012 employees were installing roofing materials on a low sloped roof and were not protected from falling approximately eight feet through a skylight to the interior lower level. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) Moravian House Antiques, Roof - On or about September 24, 2012 an employee was unrolling new rubber roofing materials near an unprotected edge of a low slopped roof and fell approximately 29 feet to grade. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
