29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Construction Site: On or about September 25, 2012 and at times prior thereto, employees were exposed to head injuries from falling items such as but not limited to, hand tools and roofing material walking, doing site clean-up, and preparing equipment and materials to go on the roof, directly under the eaves of the roof tear off and shingle work.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: a) Construction Site: On or about September 25, 2012 and at times prior thereto, employees were exposed to fall of approximately nineteen feet to the ground below while tearing off old, and applying new asphalt shingles on a 4:12 pitch roof without fall protection.

29 CFR 1926.503(a)(1): The employer did not provide an effective training program for each employee who was exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: (a) Construction Site: On or about September 25, 2012 and at times prior thereto, employees were exposed to fall of approximately nineteen feet to the ground below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
