29 CFR Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards and crushed-by hazards associated with the use of an unapproved work platform attached to the forks of a rough terrain forklift: a) On or about 7/31/2012, at a two story residential home located at 2102 Calumet Ave. New Holstein, WI 53061, employees were exposed to fall hazards and crushed-by hazards associated with the unapproved work platform potentially collapsing or causing the rough terrain forklift to overturn while performing remodeling work. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
