29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) Pole shed addition at Hixwood Metal, Inc. Stanley, WI; workers installing steel sheeting to the structure were not using an effective means of fall protection and exposed to fall hazards up to 13 feet while on a work platform that was elevated by a boom lift.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift (Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e)): (a) Pole shed addition at Hixwood Metal, Inc. Stanley, WI; a worker was installing steel sheeting to the structure from an aerial lift and was not using a body belt or personal fall arrest system attached by a lanyard to the boom or basket.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) Pole shed addition at Hixwood Metal, Inc. Stanley, WI; workers using a work platform on a boom lift and an aerial lift to attach metal sheeting to the celling of a pole shed were not trained in associated fall protection hazards.
