29 CFR 1926.304(i)(1):   Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:    Jobsite:  The Dewalt circular hand fed rip saw that was used by employees to rip/cut cedar shingles was not guarded with a hood that completely enclosed the portion of the saw above the table.

29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Jobsite:  The employees who were working from the fabricated metal frame scaffold platforms at heights of approximately 10 feet 2.5 inches above the ground were not protected from fall hazards by the use of guardrails or personal fall arrest systems.

Jobsite:  The employer did not ensure that each employee that was involved in erecting and maintaining the welded frame scaffold was trained in the procedures and recommendations for the safe erection and inspection of the scaffold that was used by the employees to perform roofing work.
