29 CFR 1926.501(b)(14):   Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    Jobsite  The exterior walls of the structure did not meet the requirements of guarding and exposed employees to fall hazards of up to and including 33 feet to the ground.

29 CFR 1926.1052(c)(1):   Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    Jobsite; The GC did not ensure that there was guard / hand rails on the exterior stairway systems that employees were using to get to the higher levels and exposed to falls of up to 57 feet 3 inches.

29 CFR 1926.1052(c)(7):   The top edge of a stairrail system served as a handrail and the height of the top edge was less than 36 inches (91.5 cm) from the upper surface of the stairrail system to the surface of the tread, in line with the face of the riser.    Jobsite;  The temporary wire rope at a height of 26 inches on the exterior stairwells that was used for the combination stairrail / handrails was not the required height to serve as either a stairrail or handrail.

29 CFR 1926.1052(c)(12):   Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926:     Jobsite  The 32 landings of the 4 exterior stairwells that the employees were using to walk up to the higher levels did not have guard rails that meet the requirements of 42 inches + or  3 inches.    Note:  Guarding of landings for stairways and ladders are being maintained under Subpart M.
