29 CFR 1926.962(b)(3): For any employee working transmission and distribution lines or equipment as deenergized, the employer did not ensure that the lines or equipment were deenergized under the provisions of 1926.961 and did not ensure proper grounding of the lines or equipment as specified in paragraphs (c) through (h) of this section. Worksite: The employer did not ensure that distribution lines that the employee was required to handle were de-energized and grounded where the hazard of induced voltage was present. On or about 12/22/2014, an employee sustained an electrical shock while handling and removing a wire that was not de-energized.
