29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    (a) Facility, 131 Kline Avenue, Callery, PA:  On or about 10/02/12 all energy isolating devices that control the guillotine on the Rondo line were not operated in such a manner as to isolate the machine from its energy sources.  An employee removed the cookie punch from the guillotine head without powering off the equipment and isolating it from its energy source.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) Facility, 131 Kline Avenue, Callery, PA:  On or about 09/27/12 the point of operation on the Rondo line guillotine, equipped with a cookie punch, was not guarded to prevent employees from having any part of their body in the danger zone during operating cycle(s).  An employee suffered a laceration and fracture to the ring finger on their left hand when dewebbing the dough and contact was made with the moving guillotine/punch.

29 CFR 1910.333(c)(10): An interlock system was not returned to its operable condition when the work was completed:    (a) Facility, 131 Kline Avenue, Callery, PA:  On or about 10/02/12 an interlock system on the rear guard on Rondo guillotine did not function properly after testing an automatic "dwebber" on the line.
