29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Facility, 131 Kline Avenue, Callery, PA: On or about 10/02/12 all energy isolating devices that control the guillotine on the Rondo line were not operated in such a manner as to isolate the machine from its energy sources. An employee removed the cookie punch from the guillotine head without powering off the equipment and isolating it from its energy source.

An employee suffered a laceration and fracture to the ring finger on their left hand when dewebbing the dough and contact was made with the moving guillotine/punch.
