29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Facility, 131 Kline Avenue, Callery, PA: On or about October 19, 2016, an employee reached under the fixed metal guard on the AMFEC Mixer to pull some remaining dough out from the unloading door and was struck by the rotating blade that was pushing the dough out and lost one finger and severely damaged two others. The Byrnes & Kiefer Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1919.212(a)(3)(ii), which was contained in OSHA inspection number 665119, citation number 1, item number 2 and was affirmed as a final order on November 30, 2012, with respect to a workplace located at 131 Kline Avenue, Callery, Butler, PA 16024.
