OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard:    Inside of the Tool Room, the following machines were not equipped with emergency stops:      a/  Bridgeport Milling Machine, Series I.  b/  Walker-Turner Drill Press, s/n 2072646.  c/  Walker-Turner Drill Press, s/n 2072646.    One method of abatement to correct this apparent hazard is to install emergency stops as required by ANSI B11.8-2001, paragraph 6.12.3. Inside of the 221 Three-Phase Tank Department, each two-hand device was not designed and constructed to protect each hand control against unintended or inadvertent operation for the following machine(s):    a/  Cincinnati 21174 Small Parts Press.    One method of abatement to correct this apparent hazard is to protect each hand control against unintended or inadvertent operation by installing ring or palm guards or other fabricated shields as required by ANSI B11.19-2010, paragraph 8.4.1.1. One method of abatement to correct this apparent hazard is to arrange each hand control so that both hands are required for actuation as required by ANSI B11.19-2010, paragraph 8.4.1.2. Inside of the 111 Core Fabrication Department, the following machines were inadequately guarded:    a/  Colt Automation Feeder #1.  This machine is equipped with two feeders.  b/  Colt Automation Feeder #2.  This machine is equipped with two feeders.  c/  Colt Automation Feeder #3.  This machine is equipped with two feeders.  d/  Colt Automation Feeder #4.  This machine is equipped with two feeders.  e/  Colt Automation Feeder #5.
