29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage: a) On or about October 3, 2012, and times prior thereto, in the Maintenance Shop; employees used a Black & Decker bench grinder for off-hand grinding that did not have a work rest installed to support the work. b) On or about October 3, 2012, and times prior thereto, in the Maintenance Shop; employees used a Jet pedestal grinder for off-hand grinding that had the work rest adjusted 1 inch from the abrasive wheel.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel: a) On or about August 15, 2012, and times prior thereto, in the Maintenance Shop; employees used a Black & Decker bench grinder that did not have an adjustable tongue guard installed. b) On or about August 15, 2012, and times prior thereto, in the Maintenance Shop; employees used a Jet pedestal grinder that did not have an adjustable tongue guard installed.
