29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: On or about October 3, 2012 and times prior thereto, employees of Menzie Contractors, LLC were observed working from a wood frame scaffold and work platforms while framing the Summit Bank Building located at or near 700 Salem Road in Conway, Arkansas. The wood frame scaffold and work platforms used were not designed by a qualified person. The load ratings for the scaffolding had not been determined, exposing employees to the hazard of falling in the event of collapse of the platforms. The scaffold platforms were located in the back of the building along the drive-through area of the bank building and on the left side facing the building.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: On or about October 3, 2012 and times prior thereto, employees of Menzie Contractors, LLC worked from a wood frame scaffold and work platforms while performing framing of the Summit Bank Building located at or near 700 Salem Road in Conway, Arkansas. The wood frame scaffold and work platforms used by the employees were not at least 18 inches wide as required. The scaffolding platform on the left side of the building (facing the building) was seven (7) inches wide. The scaffolding platform at the back of the building was twelve (12) inches wide. These conditions exposed employees to the hazard of falling from an elevation.
