29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about November 30, 2012, at the job site located at the intersection of Arch and Baseline in Little Rock, Arkansas, where employees were erecting trusses and decking the roof for a new church building, the employer did not ensure the employees use fall protection, exposing the employees to the hazard of falling from heights of 13 1/2 to 42 feet to the ground below.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about November 30, 2012, at the job site located at the intersection of Arch and Baseline in Little Rock, Arkansas, the employer did not ensure a portable ladder, employees used to access the roof, extended at least 3 feet above the upper landing surface, exposing employees to the hazard of falling approximately 13  1/2 feet to the ground below.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee who might be exposed to fall hazards:    On or about November 30, 2012, at the job site located at the intersection of Arch and Baseline in Little Rock, Arkansas, the employer did not provide a training program for employees exposed to fall hazards. As a result, employees were exposed to the hazard of falling from heights of 13 1/2 to 42 feet to the ground below.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:    On or about November 30, 2012, at the job site located at the intersection of Arch and Baseline in Little Rock, Arkansas, the employer did not provide training to employees who used ladders to perform their duties. As a result, employees used a ladder to access/egress the roof that did not extend at least 3 feet above the upper landing surface, exposing employees to the hazards of falling, exposing employees to the hazard of falling approximately 13  feet to the ground below.
