Employers are required to provide fall protection for employees erecting or dismantling supported scaffolds where the installation and use of such protection is feasible and does not create a greater hazard:    a) On or about October 2, 2012 at the jobsite located at 2127 W. 22nd Place, Chicago, IL. - employees erecting a tubular welded frame scaffold were not using personal protective equipment, including but not limited to harnesses and guardrails, thereby exposing the employees to falll hazards of approximately one hundred (100) to one hundred and fifty (150) feet.
