29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about October 3, 2012 and times prior thereto, employees of Rays Plastering, LLC worked on foam insulation at the building steeple while on the roof of the Summit Bank building being constructed at 700 Salem Road in Conway, Arkansas.  Employees performing work associated with the application of an exterior engineered insulation system (EFIS), were not protected from falling from the steep sloped roof with a slope of 8:12, exposing them to a falling distance of greater than 6 feet.
