29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about October 4, 2012, and at times prior, for employees working on or near unprotected sides and edges and exposed to a fall of approximately 16 feet from eave-to-ground on a steep-sloped roof while engaged in re-roofing activities without fall protection systems in place at Avera Medical Center, located at 525 North Foster Street in Mitchell, South Dakota.
