29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a.At the site, employees were exposed to a 14 feet fall hazard, in that, a tubular welded frame scaffold two frames high with a two foot trestle scaffold on the top frame did not have the working platform fully planked as required. Two 19 inches wide prefabricated planks were used on the 60 inches wide scaffold frame.

29 CFR 1926.451(f)(7):  Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:    a. At the site, employees were exposed to a 14 feet fall hazard, in that, a tubular welded frame scaffold two frames high with a two foot trestle scaffold on the top frame did not have guardrails and was not provided with a fully planked working platform. The scaffold was erected, moved and altered without the supervision of a competent person knowledgeable on scaffold safety.

29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards as required by paragraph (a)(1)-(5) of this section:    a. At the site, employees were in progress of sheetrock installation and were using a tubular welded frame scaffold two frames high with a two foot trestle scaffold on the top frame without guardrails and without having the working platform fully planked.    Employees involved in scaffold use were not provided training and education in the following:    1. The nature of fall hazards and any falling object hazards.    2. The correct procedures for dealing with erecting, maintaining, and disassembling the fall protection system and falling object protection system being used.    3. The proper use of the scaffold, and the proper handling of material on the scaffold.    4. The maximum intended load and load carrying capacities of the scaffold.    5. Any other pertinent requirements of Subpart "L" (Scaffolds) under 29 CFR 1926.

Employees involved in scaffold erection were not provided training and education in the following:    1. The nature of scaffold hazards;    2. The correct procedures for erecting, dismantling, moving, operating, repairing, inspecting and maintaining the scaffold used;    3. The design criteria, maximum intended load-carrying capacity and intended use of the scaffold;    4. Any other pertinent requirements of Subpart "L" (Scaffolds) under 29 CFR 1926.
