29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    Instance 1) At the jobsite, east side of residence, the base of the tubular welded frame scaffolding was not adequately supported, in that, base plates were not installed on the scaffold legs to provide a firm foundation.    Instance 2) At the jobsite, north side of residence, the base of the tubular welded frame scaffolding was not adequately supported, in that, mud sills were not installed on the scaffold legs to provide a firm foundation.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    Instance 1) At the jobsite, east side of residence, an employee was exposed to fall hazards up to 19'2" while climbing up the end section of the through style tubular welded frame scaffolding, in that, an adequate means of access and egress was not provided.     Instance 2) At the jobsite, north side of residence, employees were exposed to fall hazards up to 12'8" while climbing up the end section of the through style tubular welded frame scaffolding, in that, an adequate means of access and egress was not provided.

29 CFR 1926.451(g)(1):     Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    Instance 1) At the jobsite, east side of residence, an employee in the process of plastering was exposed to a 19'2" fall hazard in that, guard rails were not installed on the top work platform on tubular welded frame scaffolding, 3 levels high.    Instance 2) At the jobsite, north side of residence, employees in the process of plastering were exposed to a 12'8" fall hazard in that, guard rails were not installed on the top work platform on tubular welded frame scaffolding, 2 levels high.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) At the jobsite, east side of the building, an employee in the process of plastering work while standing on a steep roof, 6 in 12 pitch, was exposed to a 9'4" fall hazard, in that, fall protection equipment was not installed or utilized. Observed on or about 1/29/13.
