On or about October 10, 2012, at the above addressed jobsite, the employer did not maintain exposure records as follows for employees conducting grinding operations: 1. Air sampling data did not contain the type of respiratory protective devices worn. 2. The job classification of the employees monitored and all other whos exposure the measurement is intended to represent. 3. The environmental variables that could affect the measurement of the employees exposure.
