29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a. On or about November 4, 2015, at the above addressed jobsite, employees were exposed to falls while performing demolition work from a tubular welded frame scaffold, approximately twenty-four (24) feet above the ground that did not have the working level fully planked.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: a. On or about November 4, 2015, at the above addressed jobsite, employees were conducting demolition operations from a chimney and scaffold approximately twenty four (24') feet above the ground and had lifelines secured to the scaffold, thereby exposing the employees to fall hazards.
