29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) Employees were exposed to trip and fall hazards where there was a break in elevation of about 4 feet between the courtyard and the main roof and no means of access and egress had been provided. No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Main Roof - Employees were exposed to the hazard of falling approximately 12 feet from the roof edge while engaged in roofing activities without the use of fall protection or safety monitoring. b) Galley Roof - Employees were exposed to the hazard of falling approximately 11 feet from the roof edge while engaged in roofing activities without the use of fall protection or safety monitoring. Roofs, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(10) which was contained in OSHA inspection number 311246342, citation 1, item 1, issued on October 18, 2007, and became final order on or about November 12, 2007. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems: a) Galley Entry Roof - Employees were exposed to the hazard of falling approximately 11 feet from the edge of a roof deck while manually hoisting/lowering materials from the edge of the deck. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
