On or about August 6, 2012, in the mechanic shop, the 28-foot fiberglass Keller extension ladder had at least two bent rungs, exposing employees to a 10 feet fall hazard. b. On or about August 6, 2012, in the paint shop, the 6-foot step ladder used to prepare vehicles had a broken leg and railing, exposing employees to a 5 feet fall hazard. c. On or about August 6, 2012, in the mechanic shop, the 6-foot platform ladder was missing the guard rail at the platform landing, exposing employees to a 5 feet fall hazard.
