On or about July 1, 2014, above the dryer in front of the electric planner in the Wash Bay, the 6-outlet flexible surge protector power strip was not used in accordance with instructions included on the labeling and it had two temporary flexible cords plugged into it, to energize a small vacuum and a fan, a third cord plugged into it to energize the dryer, a fourth cord plugged into it for powering a microwave, and a fifth cord plugged into it for a radio, exposing employees to an electrical shock hazard.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a. On or about July 1, 2014, an employee was required to wear a SAS dual cartridges respirator with AOSafety organic cartridges while conducting painting tasks. The employee was also required to wear 3M dust mask while performing sanding activities in the spray booth. The employer had not established a written respiratory protection program to ensure that the employee was medically able to use that respirator and fit tested and proper trained on respirator use, maintenance, and care of the respirator.
