29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): BX Solutions, Inc., located at 1 Air Cargo Parkway East, Swanton, OH 43558: On or about October 9, 2012, the employer failed to certify that all employees, operating powered industrial vehicles such as, but not limited to, LOG 03 forklift (Prime Mover, Model RC40B, S/N RC40B350), have been properly trained to complete the task, and their performance in safe operation of powered industrial vehicles in the facility have been evaluated. No additional abatement information is needed for this item.
