29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area: Lineage Logistics Holdings, LLC, Perry, GA - Employees working in the warehouse were exposed to carbon dioxide from dry ice in some of the boxes containing poultry products. The training did not contain the health hazards of carbon dioxide on or about 10/9/12. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1020(g)(1): Upon first entering into employment, and/or at least annually thereafter, the employer did not inform current employees of the existence, location, and availability of any records covered by 29 CFR 1910.1020; the person responsible for maintaining and providing access to records; and each employee's rights to access to these records: Lineage Logistics Holdings, LLC, Perry, GA - Employees working in the warehouse were exposed to carbon dioxide from dry ice in some of the boxes containing poultry products. Monitoring had been conducted by the employer using a direct reading instrument. Employees had not been informed of the requirements under 1910.1020(g)(1), on or about 10/9/12. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
