On October 09, 2012 an employee, who was not wearing a personal fall arrest system, fell 19-feet 8-inches from a steep, 8 in 12, pitched unprotected roof at the rear of the residence while installing roof sheathing.

29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: a) Roof of residence. An employee, installing roof sheathing on a steep, 8 in 12 pitched roof, 19 feet 8 inches above ground level, was not provided an adequate anchor attachment point for his personal fall arrest system. The lifeline, which had been wrapped around the roof ridge beam as an anchor point, was not protected from being cut or abraded.
