29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) Front side, new residential construction located at 13494 Hipworth Road, Milton, GA 30004: Employee was exposed to severe lacerations and amputations while operating a Makita (Model - 5007F) 7-1/4-inch Circular Saw without a lower guard. The operator of the saw used a wood shim between the upper and lower guard to prevent the lower guard from retracting after a cut was made which exposed the employee to injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Second floor, new residential construction located at 13495 Hipworth Road, Milton, GA 30004: Employees were exposed to falls of approximately 14-feet to 22-feet, 8-inches while engaged in residential framing activities on the second floor as no fall protection system was in place. MB Framing & General Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 674799, citation number 1, item number 1(a), and was affirmed as a final order on 03/28/2013, with respect to a workplace located at 2711 Vining Orchard Circle, Smryna, GA 30080.
