29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.     (a) Marathon Roofing, LLC on or about October 09, 2012, at the address of 237 Marina Parkway Lorain, OH: The employer did not ensure employees were protected from head injuries while working below employees conducting roofing work above. Employees were exposed to potential impact injuries caused from falling material and/or equipment.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    a)  Marathon Roofing, LLC on or about October 09, 2012, at the address of 237 Marina Parkway Lorain, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a residential type structure.  Employees were exposed to a 29 feet fall to the pavement below.    Marathon Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 420574, citation number 1, item number 1a and was affirmed as a final order on July 10, 2012, with respect to a workplace located at 302 Canterbury Dr. Huron, OH.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    a)  Marathon Roofing, LLC on or about October 09, 2012, at the address of 237 Marina Parkway Lorain, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various roofing operations.    Marathon Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 420574, citation number 1, item number 1b and was affirmed as a final order on July 10, 2012, with respect to a workplace located at 302 Canterbury Dr. Huron, OH.
