29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:    a) Mule Parking:  On October 9, 2012 and at times thereto, the electrical power supply cord used to power the batteries on the electrical cart or mule had exposed wires and damaged insulation.  The outer sheathing insulation was pulled away from the wires and created a hazard to employees when used.
