29 CFR 1926.152(a)(1):    Containers and portable tanks used for the storage and handling of flammable and combustible liquids were not approved:    a.   On or about October 10, 2012, at the northwest side of the house, a damaged portable tank was used for storage and handling of gasoline used to fuel the air compressor.

29 CFR 1926.501(b)(13):    Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a.   On or about October 10, 2012, on the front of the house, an employee installing framing did not use an adequate anchor point for his personal fall arrest system, exposing himself to a 9 feet fall hazard.    b.   On or about October 10, 2012, in the trusses on the southwest side of the roof, an employee installing framing was exposed to a 14 feet fall hazard in that he was not protected by a fall protection system.    c.   On or about October 10, 2012, at the south corner of the house, an employee installing framing was exposed to a 9 feet fall hazard in that he was not protected by a fall protection system.

29 CFR 1926.1053(b)(1):    Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a.   On or about October 10, 2012, at the south corner of the house, the roof access ladder did not extend above the roof eave, exposing an employee to a 9 feet fall hazard.

29 CFR 1926.1053(b)(16):    Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:    a.   On or about October 10, 2012, inside the house, a 20-foot aluminum extension ladder that framing employees used to access the trusses was missing a pawl on the fly section.
