29 CFR 1926.451(g)(3)(iv): Vertical lifelines, independent support lines, and suspension ropes shall not be attached to each other, nor shall they be attached to or use the same point of anchorage, nor shall they be attached to the same point on the scaffold or personal fall arrest system.    a.  On or about October 09, 2012 - at the above addressed jobsite, employees were performing tuckpointing operations from a two point suspended scaffold, the vertical lifelines were attached to the same point of anchorage, thereby exposing employees to falls of approximately fourteen (14) feet six (6) inches.

On or about October 09, 2012 - at the above addressed jobsite, an employee performing tuckpointing operations from an UpRight SB60 Telescopic Boom Aerial Lift basket climbed outside the basket, thereby exposing the employee to fall hazards.

On or about October 09, 2012 - at the above addressed jobsite, employees working in an UpRight SB60 telescopic boom aerial lift were not attached to the boom or basket by the use of a lanyard, thereby exposing the employees to the hazards associated with a fall of more than twenty five (25) feet.

The training shall include the following areas, as applicable:    a.  On or about October 09, 2012 - at the above addressed jobsite, employees using the UpRight SB60 Telescopic Aerial Boom Lift were not trained to recognize the hazards associated with the use of aerial lifts, thereby exposing the employees to the hazards associated with falls.
