On or about, December 28, 2016, at the above addressed jobsite, employees conducting masonry operations from tubular welded framed scaffold, greater than twenty five (25) feet above the ground, were exposed to fall hazards due to the lack of a guardrail system at the end of the outrigger platform.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a. On or about December 28, 2016, at the above addressed jobsite, an employee conducting masonry operations from a tubular welded frame scaffold was not trained to recognize the hazards associated with scaffolding. The employee was thereby exposed to the hazards associated with falls. The Miron Masonry & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.454(a), which was contained in OSHA inspection number 679318, citation number 1, item number 3 and was affirmed as a final order on March 7, 2013, with respect to a workplace located at 2001 W. Race Ave., Chicago, IL.
