29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 10/09/2012, employees were exposed to amputation hazards from ingoing nip points while operating belt grinders and a press brake that were lacking point of operation guards to finish metal components at a fabrication facility located at 12370 SW 130 Street, Miami, FL 33186.    5 instances:   Instance A:  # 1 Burr King Belt grinder located in Landcruiser area.  Instance B:  # 2 Burr King Belt grinder located in Landcruiser area.  Instance C:  # 3 Burr King Belt grinder located in Toyota Wagon area.  Instance D:  # 4 Burr King Belt grinder located in Toyota Wagon area.  Instance E:  AccurPress brake lacked point of operation guarding while bending metal components.    NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):    On or about 10/09/2012, employees were exposed to shock and electrocution from using damaged 110 and 220 VAC extension cords and a power strip at a fabrication facility located at 12370 SW 130 Street, Miami, FL 33186.     4 instances:   A. 220 VAC shop built extension cord that was not approved for use powered a Millermatic 252 wire welder. Landcruiser area.    B. Newpoint Model AV 500 power strip with missing ground pin connected to yellow extension cord. Landcruiser area.    C. 110 VAC extension cord with damaged insulation jacket that supplied power to Newpoint Model AV 500 power strip.  Landcruiser area.    D. 220 VAC power cord with damaged insulation jacket at the plug that did not have strain relief that supplied power to Burr King Belt Grinder 3-308 (#1).  Landcruiser area.
