29 CFR 1918.62(b)(3)(ii): Wire rope or wire rope slings having kinking, crushing, bird caging, or other damage resulting in distortion of the wire rope structure were used: a) The employer does not ensure that wire rope with kinking, crushing, bird caging, or other damage which distorted the wire rope structure is not used. This violation occurred when employees were allowed to operate a winch with one (1) inch wire rope to rig the "F Walker" vessel to AEP River Operations Barge PML264B on the Mississippi River that had kinking and other damages that distorted the wire rope structure.

29 CFR 1918.62(b)(5): Where bolt clips were used to form eyes without the manufacturer's recommendations and without the use of Table 1 of Appendix II in this section to determine number and spacing of the bolt wire rope clips: a) The employer does not ensure that U-Bolt clips adhere to manufacturers recommendations or 29 CFR 1918, Appendix II to determine the number and spacing of the clips. This violation occurred when employees were allowed to use one (1) inch wire rope with U-Bolts that were spaced less than the minimum six (6) inches as described by Appendix II.
