Worksite - The Royal Forklift (Serial number IG784LP) had been modified in that the addition of counter weights to the rear of the forklift had not been approved by the manufacturer.

29 CFR 1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch and Grinding machinery was not used with work rest(s) to support offhand grinding work:    a.  Maintenance area tool crib - The smaller bench grinder was not provided a work rest for both abrasive wheels exposing employees to amputation hazards.    b.  Maintenance Area tool crib - The larger bench grinder was not provided with a work rest on one abrasive wheel and the single work rest which was provided was not adjusted to a maximum opening of less than 1/8th of an inch width exposing employees to struck by and amputation hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch and/or the guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:    a.  Maintenance area tool crib- The smaller bench grinder equipped with 2 tongue guards  for the abrasive wheels did not have the tongue guards adjusted to the periphery of the wheel to ensure that the opening was less than 1/4 of an inch thereby exposing employees to struck by and amputation hazards.    b.  Maintenance area tool crib - The larger bench grinder left side abrasive wheel tongue guard was not adjusted so that the opening was less than a 1/4 inch thereby exposing employees to struck by and amputation hazards.    c.  Maintenance area tool crib - The larger bench grinders right side abrasive wheel was not equipped with a tongue guard.

Green line machine 1200 - The point of operation on the mechanical power press was not provided with point of operation guards thereby exposing employees to amputation hazards on the front, side and rear of the press.    b.  Green line machine 1320 - The point of operation on the mechanical power press was not provided with point of operation guards thereby exposing employees to amputation hazards on the front and rear of the press.    c.  Green line machine 1299 - The point of operation on the mechanical power press was not provided with point of operation guards thereby exposing employees to amputation hazards on the front, side and rear of the press.    d.  Green line machine 1318 - The point of operation on the mechanical power press was not provided with working point of operation guards, thereby exposing employees to amputation hazards on the front and rear of the presss.  The light curtains were not functioning.    e.  Green line machine 1198 - The guarding on the mechanical power press did not guard the point of operation.  The guards on the machine were not interlocked with the drive mechanism and stayed up during activation of the controls, thereby exposing employees to amputation hazards.    f.  Green line machine 1100 - The guarding on the mechanical power press did not guard the point of operation.  The guards on the machine were not interlocked with the drive mechanism and stayed up during activation of the controls, thereby exposing employees to amputation hazards.    g.  Yellow area machine 1163 - The point of operation on the mechanical power press was not guarded thereby exposing employees to amputation hazards.
