29 CFR 1910.67(c)(2)(v): A body harness/belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) On or about 10/09/12, on jobsite at Burger King Store, 6375 Brockport-Spencerport Rd., Brockport, NY: An employee was exposed to potential fall of approx. 20 ft. while changing light bulbs in light fixtures attached to the top of a pole. The employee was inside the elevated "basket" of a "JLG Model T350" aerial lift, but was not wearing a body harness/belt with a lanyard, and was not tethered to the basket in any other way. NO ABATEMENT CERTIFICATION REQUIRED
