29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. On or about October 24, 2012 and at times prior there to, the employer did not ensure that endrails were installed on pump jack scaffolds. Employees were exposed to falls of approximately 12 feet to the ground below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 24, 2012 and at times prior thereto was exposed to a fall of approximately 10 feet to the ground below while walking on an unguarded balcony. No fall protection was in place or worn.
