29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) Gazebo Roof - On or about August 29, 2013 one employee was using pneumatic air nail gun to install roof sheating. Employee was not wearing safety glasses. Employee was exposed to being struck by flying debris or nails.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Gazebo Roof - On or about August 29, 2013 one employee was working on a roof installing roof sheathing on a 3:12 pitch roof with unprotected sides. Employee was exposed to falling 8 feet 11 inches to a lower level. Employee was not protected from falling to a lower level by the use of personal fall arrest system, safety net system or guardrail system.
