29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. a) In the mixing area, the mixing operator and other employees were exposed to an 9 foot 6 inch fall hazard due to only a single 35 inch high guardrail in use along the walkway where the skip hoist pit area is accessed and cleaned.

Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On or about October 5, 2012, a laboror was exposed to pinch point hazards when attempting to enter or look into the building where cubes of block on a conveyor are pushed out of the building through a wall opening by the cuber machine. b) In the mixing area, the mixing operator was exposed to rotating mixing blades due to inadequate and no guarding in place where dry product and water are added through openings into the mixing tank.
