29 CFR 1926.304(i)(1): Each circular hand-fed ripsaw shall be guarded by a hood which shall completely enclose the portion of the saw above the table and that portion of the saw above the material being cut. The hood and mounting shall be arranged so that the hood will automatically adjust itself to the thickness of and remain in contact with the material being cut but it shall not offer any considerable resistance to insertion of material to saw or to passage of the material being sawed. The hood shall be made of adequate strength to resist blows and strains incidental to reasonable operation, adjusting, and handling, and shall be so designed as to protect the operator from flying splinters and broken saw teeth. It shall be made of material that is soft enough so that it will be unlikely cause tooth breakage. the hood shall be so mounted as to insure that its operation will be positive, reliable, and in true alignment with the saw; and the mounting shall be adequate in strength to resist any reasonable side thrust or other force tending to throw it out of line.      WORKSITE: The Ryobi 10 table saw, used by employees for ripping lengths of medium density exterior grade fiber-cement vertical siding panel was missing the following:    1. A hood or equivalent guard the upper guard that would enclose the saw blade above the table and above the material being cut.    2. A spreader device to prevent material from squeezing the blade.    3. An anti-kickback device.

29 CFR 1926.451(c)(3): Supported scaffold poles, legs, posts, frames, and uprights shall be plumb and braced to prevent swaying and displacement.    WORKSITE: The vertical poles on the Alum-a-Pole pump jack scaffold which measured approximately 38 feet in length were not supported as necessary to prevent swaying and/or displacement.

29 CFR 1926.451(e)(8): Direct access to or from another surface shall be used only when the scaffold is not more than 14 inches (36 cm) horizontally and not more than 24 inches (61 cm) vertically from the other surface.    WORKSITE: The space between the Alum-a-Pole pump jack scaffold and the Werner 16-foot aluminum extension ladder measured 18 inches, without a handhold or similar device, exposing the employees to a 10 foot fall while accessing/egressing the working platform of the scaffold.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    WORKSITE: The employees cutting medium density exterior grade fiber-cement vertical siding panels with power saws, and nailing the sheets of vertical siding panels with pneumatic nail guns were not wearing eye protection.    G. S. Construction LLC was previously cited for a violation of this occupational safety and health standard (29 CFR1926.102(a)), which was contained in OSHA inspection number 313739161, citation number1, item number 1, affirmed as a final order on 7/9/2012, with respect to a workplace located at 4 Dover Street, Norwalk, CT 06850.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    WORKSITE: The employee installing siding from an Alum-a-Pole pump jack scaffold that was missing adequate fall arrest system, exposing the employee to a 17 foot fall.    G. S. Construction LLC was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.451(g)(1)(i), which was contained in OSHA inspection number 313739161, citation number1, item number 5, affirmed as a final order on 7/9/2012, with respect to a workplace located at 4 Dover Street, Norwalk, CT 06850.
