29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): The employer failed to ensure that employees conducting end of car unit removals were aware of and followed the Job Safety Analysis (JSA) procedures developed by the company. A JSA was developed for use companywide. Workers and managers were burned when a welder was cutting a stop off of an end of car unit to remove it. During the cutting process a piston released. Oil mist was released into the air igniting into a fire ball burning 2 workers standing in the area. Had the employee been aware of the proper procedures they would have been wearing the proper PPE to protect them from their injuries.
