OSH ACT of 1970 Section (5)(a)(1):     Section 5(a)(1) of the Occupational Safety and Health Act of 1970:    The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to explosion and/or burn hazards.      The employer has failed to ensure the personnel performing maintenance activities on direct fired liquid petroleum gas vaporizer units were knowledgeable as to the manufacturers requirements of their use and maintenance.   The lack of knowledge allowed an overpressure situation to occur during a purging process.  The released gas was then ignited creating a flash fire which injured two maintenance personnel.      AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS:    (a)  Ensure that only qualified and knowledgeable personnel perform any operations, to include maintenance, on the direct fired LP gas vaporizer units.     (b)  Conduct training with any personnel that work with the LP gas vaporizer units which includes a review of the operators manual and specifically any warnings contained therein.  This training would include any of the manufacturers required steps for the operation of unit.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

At the following locations, the employer failed to ensure that the hoists were inspected in a manner that met the requirements outlined in ANSI B30.16.  Specifically, the quarterly inspections did not go beyond a visual inspection of the hoist housings and excluded the internal components.      a)	G-Dryer Room; CM 1 ton hoist    b)	 2nd Floor Cooker Room; 2 CM hoists    c)	 Maintenance Shop; Robbins & Meyers Inc hoist      AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS:    (a)  Ensure that maintenance personnel are trained on the requirements for hoist inspection outlined in ANSI B30.16.       (b)  Conduct hoist inspections as outlined in the ANSI standard at the prescribed intervals to ensure that the hoists are maintained in a safe and serviceable condition.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.
