29 CFR 1904.7(b)(3): The employer did not correctly classify/enter the following work-related injuries or illnesses on the OSHA Form 300 or equivalent. The Highland Living Center at Pittsford Dialysis Area - On or about 10/17/12, the employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2012. In March, 2012, an employee sustained an injury while pumping acid. The employee was transferred or restricted for a period of time from their regular job duties. The employer had not entered the employee's name, the date of the injury, the description of the injury (parts of the body affected, object/substance that directly injured the employee), or the actual number of days the employee was transferred or restricted from their regular duties. ABATEMENT CERTIFICATION REQUIRED

The Highlands Living Center Dialysis Area - On or about 10/17/12, the employer forms used for recording injuries and illnesses were not an equivalent form for the calendar year 2012. The employer did not use the OSHA form 301, or equivalent to provide ancillary information about an injury sustained in March, 2012 resulting from pumping acid. ABATEMENT CERTIFICATION REQUIRED
