29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about 10/16/2012 at 481 Northolt Parkway Suwanee, GA; employees performing framing operations were exposed to electrical hazards where grounding pins were missing from the 110 V extension cords. 1. Building 400 Abel Radilla, sub-contractor, using (2) extension cords powering (2) Makita circular saws and (1) DeWalt Drill 2. Building 600 Samuel Garcia, sub-contractor, using (1) extension cord powering a circular saw on the upper level.
