29 CFR 1910.215(a)(4):     Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    (a) Work Shop - On or about 10/17/12, employees were exposed to struck-by hazards when the work rest on a bench grinder was not adjusted to within 1/8 of an inch.     The Department of Veterans Affiars was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.215(a)(4) which was contained in OSHA inspection number 301510780, citation number 03, item number 001 and was affirmed as a final order on 7/19/10 with respect to a workplace located at 1310 24th Ave, Nashville, TN 37212    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    (a) Work Shop - On or about 10/17/12, employees were exposed to struck-by hazards when the tongue guard was not adjusted to 1/4 of an inch.    The Department of Veterans Affiars was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.215(b)(9) which was contained in OSHA inspection number 301510780, citation number 03, item number 002 and was affirmed as a final order on 7/19/10 with respect to a workplace located at 1310 24th Ave, Nashville, TN 37212    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(k)(6):     The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:    (a) Mountain Home National Cemetery - On or about 10/17/12, employees wearing North N95 dispoable respirators were not trained on the information presented in Appendix D of the respiratory protection standard.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a) Work shop - On or about 10/17/12, the employer did not ensure that the eyewash stations were ready for emergency use by properly conducting and recording eyewash checks and protecting nozzles from airborne contaminants as outlined in ANSI Z358.1    The Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.151(c), which was contained in OSHA inspection number 312670854, citation number 01, item number 002 and was affirmed as a final order on 01/06/2009, with respect to a workplace located at 10000 Bay Pines Blvd, Bay Pines, FL 33744    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
