29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    Employees were using nail guns to nail shingles on the roof and use circular saws to cut roof deck without safety glasses at the following locations within Oxford Hills Apartment Complex:      a) At 1229 Oxford Hill Court, St. Louis, MO, 63146, three employees were exposed to a struck-by hazard while they operated nail guns without donning the appropriate eye protection.        b) At 1226 Oxford Hill Court, St. Louis, MO, 63146, five employees were exposed to a struck-by hazard while they operated nail guns without donning the appropriate eye protection .        c) At 1226 Oxford Hill Court, St. Louis, MO, 63146, an employee was exposed to a struck-by hazard while he operated a circular saw without donning the appropriate eye protection.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     The employer allows employees to be exposed to a fall hazard created by performing work activity from the roof of the residential apartment structure without properly installing fall protection systems.  The most recent occurrences were at 1229 Oxford Hill Court, St. Louis, MO, 63146 and 1226 Oxford Hill Court, St. Louis, MO, 63146, where workers were observed on top of the two 5:12 pitch roofs, approximately 17-25 feet above ground, wearing safety harnesses with no lanyards attached to an anchor point.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Five employees gained access and egress by utilizing a portable ladder which did not extend at least 36 inches above the upper landing (roof).  The side rails measured approximately 24 inches above the landing.  This improperly positioned ladder was located at the roof eave along the east side of 1226 Oxford Hill Court, St. Louis, MO, 63141.  The fall hazard at the ladder landing measured approximately 17 feet, 11 inches.
