29 CFR 1926.404(b)(1)(iii): Assured equipment grounding conductor program. The employer shall establish and implement an assured equipment grounding conductor program on construction sites covering all cord sets, receptacles which are not a part of the building or structure, and equipment connected by cord and plug which are available for use or used by employees. This program shall comply with the following minimum requirements: (a). Worksite located at 2100 E. 1st Street, Maryville, Missouri- Employees were exposed to a shock or electrocution hazard in that the extension cord was not provided with a GFCI, nor was the cord provided with a grounding pin.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. (a). Worksite located at 2100 E. 1st Street, Maryville, Missouri- An extension cord was run through a metal doorway and was pinched by the doorway and could breach the insulation and cause the doorway to become energized.
