29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: On or about 10/18/12 - 1491 Melton Rd, Hoover, AL, an employee was in the excavation on the west side of building 2300 and identifying where to dig so the backhoe operator removing dirt would avoid hitting the underground installations.
